All our services are planned and carried out in accordance with a service delivery plan, which will be agreed with you at the outset and may be reviewed at any time. Churchfields Home Care aims to provide services that meet your needs and achieve the desired outcomes.
All service delivery plans are made with regards to any health & safety risks to either your carer or yourself of any activity. Any aids which may be necessary to fulfil tasks will be stipulated in the plan and will only be used by trained, competent care staff.
The service provided is based on assessed need and preferences and value based, person centred, and outcome focused and includes various support facilities.
